I have always liked Sbarro best, and they make great NY style pizza IMO. Unfortunately, there isn't one very close to me, so I'm stuck with Pizza Hut.

If he told you it was out of their district when you placed the order you probably should have tipped more. Well actually, the manager should have added it to the order price as a delivery fee. It is a huge pain in the ass to drive to a place that's way out of range, only to get a shit tip. That costs not only more gas, but time where you could probably be delivering 4-5 other orders closer to the shop-- which can mean a lot of money lost for the driver. Depending on how far from the store it was, $5 really might not have been worth it for the driver.
I say all this because I used to deliver. for PJ's, actually, as well as a couple other places. Usually if I was going way the fuck out of their normal delivery range (like a couple towns over) the tip would be closer to $10-$20.
Bottom line is: If you don't have the money to give the driver a decent tip, you should be picking up the pizza yourself. I'm just stating the point-- not saying this was your fault. Like I said, the manager should have made all of this very clear at the time you placed the order. If he didn't, this is on him. If he did, it's kind of on you.

Incorrect. Restaurants that offer delivery are not required to deliver to any location. Quite the opposite. Delivery places normally have a very limited, clearly defined range of where they will and will not deliver. This is for a number of simple and very practical reasons-- mainly because it takes way more time and gas to go far from the store, and not only would the store need to hire more drivers, bringing up their labor costs, but the drivers would hate having to drive miles away from the store for douchebags who are both too lazy to pick up their own food, and too cheap to pay fairly for the service.
So typically when a customer calls from outside the delivery range, they are flat-out refused delivery. If it's a particularly slow night, a particularly large order, or a particularly good / important customer, the manager will sometimes make an exception. They do not have to do this. It is a favor and an extra service. It is not unusual in the slightest for the manager to require a delivery charge be paid for this additional service, even if such a charge is not part of their normal operation.
As I said-- the manager should have made this very clear before the order was agreed upon, and if he didn't then it is a bit of a dickhead move. Even if he didn't require a delivery charge, however, but did alert the customer that it's outside their normal delivery area, one (the manager) might expect that the customer would realize that this company, and more specifically the driver, is doing them a favor, and that people were going above and beyond for them, and that it would only be decent of them to do likewise, in regard to compensation. $5 would be a good tip in town, closer to the store. Like I said-- I've seen tips going from $10 up to $50 or greater for way-out-of-range deliveries, though $10-$20 is far more common.
I don't think it's necessarily 100% uncalled-for, for the manager to call and say "hey, the driver did you a favor, and that is not the standard rate for such a favor..." Otherwise the customer might call back again to try to get the same service, not realizing the driver felt somewhat cheated on the deal, last time.
I don't think it's super-unusual, either. If memory serves, IME usually when I got back from such a run, the manager would always ask me if the tip was okay, to be sure I was treated fairly. Though I suppose this could have just been to determine how to handle the customer in the future. Either way, I'm sure you get the point....

I always yearned for the day that there would be a pizza delivery thread so I could share this story, so here goes.
Back when I was 16 I delivered pizza for a small pizza place (I think it was run by Albanian mobsters but whatever). I made a delivery to this one place -- big ass mansion crib with damn near an olympic size swimming pool in the back. They were having a HUGE fucking party out back; everybody was bombed out of their minds. Pretty sure they were stoned, too. I dropped off like 10 boxes of pizza and all these ripped out of their mind old dudes & chicks started telling me how awesome I was and how awesome pizza is and how life is just fucking grand as fuck.
They all started pulling out twenties and handing them to me. I was getting bombarded with money for delivering some damn pizzas. By the time they finally let my ass outta there I had pocketed over $300.

I bust my ass for shit pay and never get to leave the store on joy rides. Every driver that's ever worked for me has fucked around on long deliveries taking the long way and going places they shouldn't or something to that degree. Delivery drivers get paid minimum wage, but like 3.65 when they are on the road in my state (right to work state). So they live off of their tips, but if it wasn't for them using their gas and dealing with car maintenance I would tell you delivery drivers (at least in my store) don't do shit but drive around town and complain about it while they wash dishes.

In case you skipped my posts in this thread, the delivery was optional. The manager / driver didn't have to do it. They could just refuse in the future.
Second, when you call a pizza place for delivery, and give them your phone number, it's with the expectation that they may call you. Once in a while, for some reason or another, people get calls post delivery. Missing driver, "hey did you get your food yet,"...etc,... all kinds of reasons. But beyond one simple reminder call-- "hey, we don't think that's quite fair payment for the service we gave," no manager in their right mind would call back and actually "harrass" a customer about it. If they didn't like their response, they'd just be put on a "do not deliver" list, or some crap.
Third point-- go ahead and piss off the guys who bring you your food by not tipping them, or acting like a jerk and see how that turns out. I mean, in the scenario above, it was the manager that called, not the driver. The driver may have had nothing to do with that, other than to honestly tell the manager what their tip was. So you would advocate punishing someone who may be innocent and had nothing to do with the so-called "harrassment"? The guy who actually sort of got the short end in the deal?
Anyway, if you do that enough, (at least the being a jerk part) you might be denied delivery in the future. If there's no delivery charge, you can be an asshole and choose not to tip. Smaller places might ban you for that, but they also more typically have delivery charges and order minimums. So the store may not care much. Chain places like Dominoes are far more likely to not give a shit if you don't tip. But the drivers will. And you'll get a reputation. And if I know you're a chronic no-tipper and an asshole, and I have four orders in my car, guess who gets his food last?
Some guys might not be so nice. There are far worse things one could do in the private of their own car to the food of a customer who was a total jerkoff. I've never done it. I wouldn't want it done to me, so I never was willing to cross that line-- even if there was a customer or two who I felt almost deserved it. Others might not have such a strong moral compass as I.
Well, I did do it to a co-worker once though. I'm not normally much of a vengeful person. I'm usually far more forgive-and-forget. That motherfucker deserved it so much, though, and he just fucked with me at the wrong time in my life. I won't get into the particulars. But it was some of the sweetest, most fucked up revenge I ever had to watch that man literally lick my urine off his fingers as he ate his wings.

Don't piss off the people who handle your food-- It's not nice, and it's generally not a good idea.
