Neurologists vs Psychiatrists
    #18827754 - 09/11/13 01:14 PM (10 years, 6 months ago)

First and foremost, I need someone who is going to script me Adderall or pretty damn close. This sounds... messy, but I've explored alternatives and dare say I'm competent to make the judgment that it works best for me.

So I need a doctor that will cough this up without regard to my drug history and who won't get distracted by "addiction science" crap and waste time like my last doctor did.

All any doctor needs to know about me and addiction is that they need to be  careful giving me benzos and never give me Ambien. Speculating about Adderall and the "addiction pathway" is not of practical relevance, nor does it seem to have clinical soundness, or at least clinical bearing as it ought to concern me.

FURTHERMORE, I'm wanting a doctor that will work with me to really optimize what my brain can do and will help me put together a regimen to completely maximize what I'm capable of, which will probably include

-vitamins, supps
-pirecetam or noopept (barring interactions; here's where I need guidance instead of just a pill mill)
-Adderall or Dexedrine, possibly a -phenidate fetamine instead??
-an alpha-2 blocker
-a low-ish dose of lithium bicarbonate to facilitate neurogenesis

And most doctors want to just treat symptoms with as conventional (and often conservative) regimen of meds as possible, so I'm wondering if I've got a better shot with a Neurologist who would be willing to collaborate with me on focusing on the hardware, rather than focusing (and getting distracted) by things such as mood.

Could a neurologist still be adequate for if I do need some sort of mood treatment outside of my ADHD treatment/cognitive enhancement cocktail?

My ex doctor, the one I'm officially going to be doing research with this October, is hopefully going to call me this afternoon to help refer me and I'm wondering if I ought to ask more about Neurologists.

What do you guys think?

I'd want to get it through a competent psychiatrist or neurologist if I were to ever employ methamphetamine in my pharmacotherapy (brand name, US, is "Desoxyn") but I don't think I'll ever be willing to administer meth to myself as it can really damage your dopamine system, even long term.

i think maybe you should talk to a neurologist, and psychiatrists and see which one works best.
theres no way to find out until you go for it, call up a hospital and tell them what you're looking
for. i honestly dont know what you're expecting to hear from the shroomery htat you haven't all
ready.

You should most definitely call up a neurologist and ask.

When searching for a psychiatrist, I called and talked with a few on the phone before actually setting up appointments. I know neurologists can be harder to book an appointment with, and I've waited months before to see one for migraines - if this is the case, I would go talk to your GP and tell them the above concerns and ask them for a referral. If you're doing research with your old doctor I would honestly just find a new one, myself, as I wouldn't feel comfortable talking about things like that to a professional contact, personally.

I'd have to agree with the pirate, I would not let a professional study partner know and help me find everything I want to be on. Just puts you in a different light and would make the partner slightly nervous.

To answer your main question: It is called doctor shopping for a reason. It most likely won't be easy but you need to mosey around to find someone, or maybe someones, that will prescribe you all of these medicines. I don't think this one will come down to a simple A or B.

I haven't heard literally anything from the shroomery about neurologists vs psychiatrists except for what's in this thread dude...

We already decided I can't be his patient... it's a professional conflict of interest of sorts. I'm not the kind of person that's concerned with the formalities of human affairs, but you've got a good point about the professional contact thing, especially for people that may be wanting professional/corporate careers rather than to hole up in the ivory tower for their whole lives.

Old psychiatrist is going to refer me to an ADHD specialist as long as its some sort of MD that can prescribe, and if they can do that they'll also be able to address any mood stability issues outside of ADHD I may have (I'm possibly bipolar, although second opinions have said I'm not and that anything indicating this was probably just drug induced behavior, which I agree with but I'm keeping an open mind).

He was my psychiatrist for 2 years and has seen me literally psychotic off k2, he knows an awkward penis hangup issue that gets to me surrounding circumcision (is anything more awkward, really?), and has also seen the absolute worst of my drug use including quasi-overdosing.

It's inconvenient I have to resort to something that is mechanically the same as doctor shopping, although to get that regiment I was thinking may be worth trying/developing, I would be adamant about using the same doctor for all of it. I am very knowledgeable about pharmacology, but not about neurology and the brain, and I also don't think I'm knowledgeable enough that I can totally safely avoid interactions, especially if pirecetam or some supplement affected something with the pharms.

It may just be a good idea for me to say, "You know, Adderall is enough to bolster my productivity and focus so I shouldn't muddle with going beyond that", although I still think I'm going to add an alpha-2 blocker for both clarity of thought and then to mitigate the heightened blood pressure of long-term amphetamine pharmacotherapy.

Thanks so much for you guys' useful input! When my doctor calls about the referral, I'll ask his opinion too about a neurologist.

I highly doubt a neurologists will give you adderall, talking from personal experience. It took my neurologist 3 years before he would even prescribe me valium and I actually needed it at one point.

A neurologist is going to send you to a psych.

It sadly has to look that way. I've tried the non stimulant options and they're not sufficient.

I am doctor shopping ish, and most certainly pill seeking... Just not to get high.

You say the non stimulant options "aren't sufficient", but noone needs them- you WANT them. IMO you're going to have to admit that to yourself, hopefully sooner than later.

It also doesn't sound like you've done your homework to me either. I doubt you'd want to deal with lithium's side effects if you were aware of them.

I don't think its "mechanically the same as doctor shopping" it IS doctor shopping. You've already made up your mind on what you want and you asked which doctor is more likely to listen to your Rx grocery list.

I would guess its harmful to rely on, romanticize, and fantasize about drugs as much as it seems you do.

They're not, and yes: I want to nearly double my output and make it super easy to manage my life. Straterra, Wellbutrin, Tenex/Intunive did not work although I'll be taking Tenex for reasons I discussed.

I'm laughing my ass off that you think I of all people haven't done my homework... Ever read Stahl's Essential Psychopharmacology? I have.

You're dead on about lithium, which is why I'm talking a sun therapeutic dose, perhaps even of lithium ororate instead of lithium bi(polar)carbonate.

I found out what I need to know about neurologists... This thread has run its intended course.
