It's healthy. The greenish color on the cap is bruising. You can only see on that mushroom because it's either an albino or just has very little pigment.
Probably the latter after taking a closer look at it.

Wow, this is really strange, never heard of this kind of problem. The only time mine have ever fallen off is if they're starting to get a little long and I fan too hard, they can occasionally break off from the force of the wind.

Don't throw those away, there's nothing wrong with them. If there's something else you think might be a contaminant post a pic of it. The mushroom is hanging off the cake like that because it's relatively large while being connected by a relative small area. Since there's no bruising on the base I'm guessing it just grew like that and wasn't from being damaged or anything like that. It happens.
I might add if whatever the green spot you said you saw on the cake, I can't see it in the pic, does turn out be mold the mushrooms are still perfectly edible.

Most likely they are mature and heavy as hell from water weight. I have had shrooms grow so large that they couldn't stand up on their own. Also I agree with Kizzie that that one is probably showing some traits of an albino, it is very common with PE.
Always separate any potentially effected cakes isolate it in it's own chamber until you are sure it is mold or bacteria, then toss it.

Just isolate the ones with any growth, but I hear once the trich turns green it has begun to spore. I don't have enough experience to advise you on what to do next. I have only gotten trich on a poorly made mini tub when I first started years ago.
