This ever happen to anyone?

It looks like something took a bite out of the cap of the mushroom and you can see the stem. I thought this was weird but it just ended up growing where you can see through the cap. Is there a reason for this?

Yup, just random mutations. With multispore grows you can get all kinds of weird-looking fruits.

Another possibility is that something nicked or damaged the tip of the mushroom when it was a pin. Mushrooms don't heal up their wounds, so the wound would have grown bigger and bigger as the mushroom did. (Incidentally, this is why people used to say that spraying with Lysol caused mutations. It didn't cause mutations, but it did damage tissue that then continued to grow.)
