Where did I go wrong?

So I'm just wrapping up my first project. 8 three gallon buckets. MS Rye/Coir/verm (properly pasteurized). As far as I know, all of the grain used was fine, especially when considering that all of the buckets colonized. Still, four of them contaminated before the first flush and the others are starting to now. So far I've only been able to pick about 1/2 oz. dry. I've chalked some of the problem up to insufficient FAE, but I did all that I could. Two holes in the bottom just above the sub on either side (3.5 inches of sub), and 4 at the top. All holes taped for colonization and then switched out for polyfil for fruiting. Tightly in the bottom holes and loosely in the top. I was wondering, should the polyfil be loose in all of the holes for fruiting? I also used shower caps via Otto's tek and fanned frequently. Humidity was pretty consistent. Had to mist here and there (along the walls) but never had a problem with the sub drying out. I'm just not sure what went so drastically wrong. If there's anything I'm overlooking that anyone knows of or any advice that anyone has, please don't hesitate.

The polyfil should be loose. Use franks dailing in monotub tek

Did you give your spawn jars a good smell before using them? A contaminated jar could be responsible for contamination.

Hey guys, thanks for the replies. I did use Frank's tek actually, and just revisited it this morning. But if I read correctly, he said to keep the bottom holes plugged tightly and the top ones loosely. And yeah, I did smell the jars. All smelled like fresh mushrooms to me.

I think this time around I'm going to mix them one bucket's worth at a time instead of tossing it all together. Will be tedious I'm sure, but worth it if that's the problem and I'm just not catching it somehow.

This could help. I mix my trays one by one in case I let a partially contaminated jar slip by.

My buckets currently have 6 one inch holes. Does anyone think it would be better to drill a greater number of smaller holes instead?

Also, I was considering bleach bombing my grow area, but that's not necessarily a possibility. I was thinking, what if I were to build a small room out of sheet plastic within that room, and bleach bomb that instead? Would that work? Or would mold spores from the outside inevitably get in?
