Apparently aborted outdoor colonization - ideas what went wrong?

About one month ago, I took a jar that seemed to be picking up an infection, cut out the bad part, broke it into large-ish pieces, and buried it in my herb garden with some coir. For a while, it seemed to be consolidating well and when I would gently check it seemed to be forming a larger, firm, consolidated area. I checked today and, while there is still white mycelium mixed in with the soil/verm, it is completely broken apart and very loose.
I wasn't betting my future on this working out, but I'm curious if anyone has thoughts on what went wrong.
- Not enough spawn? While just one jar, I did not spread the pieces out very far at all; I filled in the gaps with coir and added that to the surrounding area; it sure seemed to be growing nicely at one point. You can fruit one jar, so why not one jar broken up and buried in a very concentrated area?
- Too much sunlight? This herb garden gets several hours of sun a day (it's a garden), but i deliberately buried this under some fairly tall, dense cilantro and mint plants. Sunlight should have been indirect or filtered at worst. Plus, I know lots of people bury sub in gardens, which generally require sunlight. Also, this has been covered by a thin layer of soil and grass clippings.
- Too hot? The average max this month has been 88, the average min 71. The hottest has been 97.
- Too much rain? We've had a couple episodes of hard rain, including the last two days. Is it possible for outdoor beds to get so heavily rained on that they just disintegrate or wash away?
- Nothing is wrong, this is part of the natural cycle? Do outdoor beds firm up, break down, and then firm up again?
Just trying to learn so that I might have success outdoors one day!

did you bury it or raised bed? if buried how deep? it could be alot of things but the majors are temp,humidity,rain etc....

tko, not sure if I follow your question -- the garden is in a 4x4 ft box situated atop the rest of the ground. I probably dug about 6" into the garden soil to bury the cake.

well it could be contamed i guess..its hard telling really. success outdoors means you have to have suitable climate,high humidity,moderate to higher temps etc... but one jar isnt much spawn for a bed that big. think of size of a mono,less than 2X2 and most use at least 4 quarts spawn...the mycelium probably just grew too weak and died. but like said hard telling.
