Innoculation question

I heard of this guy who on his 1st attempt growing, innoculated his jars this way.: 1/2 pint jars using verm and brf; made the substrate and instead of using the lid that came with the jar, he used foil and the band to tighten to get a nice seal; pressure cooked and then poked holes through foil when innoculated.
This way produced 9/10 non-contamanated cakes (So he was happy)

His next batch he did the same thing, but was unsure if contaminates could get in with the holes in the foil, so he covered the jars with a 2nd square of foil after innoculation. This produced 100% contam.

So he is wondering if the jars need a open hole while the mycellium grows?

He has read many teks but some go with lids with holes and some go with foil and the pics dont show if the holes need to be left open.

He has also heard of going: substrate, then foil, then fill the jar to top with dry verm, then a lid with holes.
Anyone think this will work for him?

I know there is a tek where you make your substrate, then make like a reservoir inside the jar over the substrate with tinfoil, then put dry verm in the reservoir cover with a lid with holes and voila. The verm keeps out contams, and you can innoc right through the tinfoil reservoir.
