SANITATION!
You probably don't have worms, but fungus maggots. Larvae of flies.
Maggots are no good, toss that jar!!! By the time that jar reaches 100% (if it does) there will be more maggots in there than you'll want to find out. Those maggots eventually mature into flies, and the cycle continues.
If you spot any flies/maggots on your birthed cakes or casings, kill them immediately (smash em). If you can get the fly before it lays its eggs, you're in good condition, if you see maggots... by the 2nd or 3rd flush, if not the first, your mushrooms and mycelia will be overrun.

Fungus flies are -really- small -- think fruit fly size. So, yeah, they could've gotten in there around the threads after you loosened it.
My advice -- either throw the jar away unopened or PC it and then trash the stuff. Don't just dump it out, since that'll leave more fungus flies in your area.

If they're not flies yet (so you don't have to worry about them flying out) it'd be easier to just take the lid off & microwave it till you're sure they're dead.
