I would say;

1. Completely waterlogged
2. Bacterial and/or mold contamination.
3. Waterlogged and bacterial and/or mold contamination.

Not that one should EVER recommend this, but a quick smell of those things would give you some info. They don't look very appetizing, I'd pick them off, put something between the cake and the perlite, and go for the next flush.

**the ones toward the top of the pic might be ok, it's tough to tell with the lighting

Everything smells great! Just like shrooms! Im thinkin water logged. Would this cause the SLOW growth? The one just to the top is in the process of doing the same thing. Tin foil, to get it off the perlite, should do it right?

If the perlite is the problem, yes, deffinitly get them on tin foil, or lids. And a good idea is to put a little moist verm under the cake, and a little on top also. Really helps growth.

I don't think waterlogged mushrooms would grow very quickly; Maybe someone who knows can shed some light.
Tin foil should do it, good luck.

OK my bud put them on tin foil last night. Holy crap did the big one explode! Today its about a third larger. Guess it was a water logged thing that was going on. Infact all the pins around it are growing much more quickly. Do you think he should pick that big one, just so it doesnt pull all the nutrients and water from the cake, there by inhibiting the growth of the smaller ones?

Well, you should pick the shroom if it is infected. If you don't pick the shroom, then it will take away nutrients from other uninfected shrooms. The shroom looks pretty nasty, if you eat it, tell us what happens. Adding to much water to perlite is a common mistake, so don't feel bad.

PS - If they are whitish foamlike masses on the cap and stem, then it is called "Weepers" and there is no real rythme or reason why it happens. From the picture it is hard to figure out what is going on, it is only apparent that it looks nasty. Need more description of sight and smell.
