In wisconsin alone ive found:
amanita amuscaria amanita pantherina ( these i kno don't count much )
one time - no proof - found paneoulus fimicola conocybe smithii pan cints gymnopilus luteofolius ( found twice in the wild ) gymnopilus luteus gymnopilus spectabilis gymnopilus sp. ( unknown )- ( weird ones that had CLOSE AS FUCK GILLS-no annulus - and bruised bright blue/green--- see these pics )  
----and dont forget pluteus salicinus
still in WI - Also found some "wild store" gymnopolis luteofolius ( looking mighty heady if i say so myself )
And ive picked (introduced) psilocybe azurenscens here in WI too
Outta state in houston TX
Psilocybe cubensis Pan cyans
Can't wait till i can add a few more woodlovers to the introduced mix
All my finds came from viewing MUCH info and pics on THIS GREAT SITE I also owe credit to the first dude introducing me to WI hunting ( actives and edibles ) he told me c.smithii should be here - along with laughing gyms - that man WAS SOOOOOOOOO RIGHT and then some  I get SOOOO much motivation from seeing all of the finds posted on here WORLDWIDE its great to be a part of this place

Only been hunting for a few months, but have found the following:
Psilocybe cubensis Psilocybe subaeruginosa Panaeolus cyanescens
Have also found Amanita muscaria (not hard where I am) and various Gymnopilus species (inactive where I live).

P. cyanescens (all along oregon coast) P. azurescens (astoria area to hammond)(why ask why try strong dry) P. semilanceata (grassy hummocks along the colombia river) P. stuntzii (gas station lawn) P. baeocytis (wood chips in portland) G. luteus (Ft Stevens Park) P. subalteatus (everywhere)
C. cyanescens (mississippi river levee area) P. cubensis (duh)

Guess I'll play too. It's kinda cool to see what other people have found. I won't count ones I couldn't ID, or the Panaeolus fimicola I find because I haven't really tried them to see if they are active
