Firstly, I started off with your average 4-tier greenhouse that was purchased on e-bay.

Then comes the essential Cool Mist impeller type humidifier. These type of very specific humidifiers are found at either a Good Will or the pharmacy isle of your local grocrey store.

Its placement is key. I've put it in the front-right corner (closer to the door). Its output vent is on the far end so it can shoot up in the GH, whereas the intake vent is hanging out the door of the greenhouse so I can achieve high levels of FAE.

Its stream of humidity needs to be uninhibited by trays. And here's why:
There is an ultrasonic (bought at Walgreens for $40) that is ran for 30 min on and 15 min off.

I have tubing from Lowe's going from the ultrasonic, run into the roof of the GH. The tubing itself is pointed at the stream of the Cool Mist. I have had great success with this and will probably keep it this way for a while.
  
Point the ultrasonic's fog right at the Cool Mist's stream, it'll be distributed throughout the GH. This may seem a bit excessive but imho a Cool Mist by itself is not adequate, yet it is essential none the less. I look at it like a base for proper RH levels.
There are a total of 4 6" Dollar Store fans, one per self. All the fans are on low and each are placed in the left corner, closer to the door. The fans are connected to an extension cord which is connected to a cycle timer that's set to run for ~30 sec. every ~10 min. This, obviously, is for ridding the CO2.
Lights are easy. They're on a 12h on/12h off timer and of course they're 6500K bulbs (purchased just about anywhere). It's hard to describe how they are mounted and how they're placed, so here's pics:

This pic was taken around the left corner of the door frame. It's where the lamp rests. The bulb is wrapped in Press and Seal so it can rest against the GH.

I think i should mention my holes because they have been carefully thought out as far as placement and size goes. All tiers have three holes each per tier. They're 2x2in in size, with the flap that I cut the holes from left in tact, spaced 2-3in apart from one another. They've been placed on the door only. Now, this alone would not be enough FAE, but with my previously mentioned fans, it's plenty!
At the bottom, I have a thick towel laid out flat to catch excess moister.
UPDATE: I put the whole thing on cinder blocks due to the fact I have carpet and it was getting really wet down there.
Anyway, I hope you enjoyed looking at this post as much as i enjoyed posting it. If you have any comments or questions, I'll be happy to respond. Any advice or editorials would be appreciated as well.
P.S. Moderately pricey, but defiantly worth the money!

Nah, but I do contemplate it sometimes due to the excess moister. As of now though I use towels.
I've really got to update this thing. Right now all I'm using is an ultrasonic and it does get wet in there. I can't stop though, I get big mushrooms!

In my green house I have found a very good way to keep the humidity and temperature pretty even throughout. My fogger pumps fog into the top right, and I have a fan on the bottom right that points straight up. This causes air to move around counter clockwise, you can see the fog swirling around in a circle. Also, get a clear drop cloth and cut a piece that fits a little larger than the bottom. Tape it around the bottom so that the green house is essentially completely sealed (doesn't have to be airtight at all). Then cut small square holes out of the green house sides at the edge of each tier and tape on a filter paper of some kind. Now your green house has maximum air circulation and filtration.
For people who use a temp/humidity controller to fully automate it, you will probably find that your fogger/fan come on for about 10 minutes every half hour. This is a good sign that you are getting plenty of air exchange through the filtered holes, and the massive air circulation from the fan is inducing pinning on a mass scale. I have had great fun with my automated green house

Just out of curiosity, what kind of fan are you using to circulate the air?

A tub of perlite, that way the evaporating water will aid in humidity.
