Habitat: Where does it grow? Eg. woods, pasture, state, province, country, altitude, etc. What does it grow on? Eg. soil, dung, wood (dead, living, what kind of wood?), etc. very old rotten cedar log and on the ground around it.
Gills: Color, attached/not, gills/pores, etc. light orange color, veined
Stem: Length, diameter, color, texture, hollow/solid, thin/thick, etc. dark brownish orangeish
Cap: Diameter, color, texture, conical/spherical, convex/concave, etc. darker in the middle lighter around the margine browniish orage fading to a yellow
Spore print color: Very important!
Bruising: Color that the mushroom bruises, if any.
Other information: Scent of the mushroom, anything else you think is important, large close-up pictures showing stem, cap and gills.maybe Xerophalina cornui ?

hab-very old cedar stump.
gills-light orange, decurrent
stem-short, light orangish yellow
cap-very small, dark orange when young, fading to a lighter orange

hab-conifer stump, probably fir
gills-white,
stem-short,darker brown at base, lighter yellow at the top
cap-small.fuzzy when young, darker brown in the middlw fading to lighter brown around the margin

and last but not least , the Blue Mycena sp.!! i really think this is a Mycena and not an Entoloma . not a 100% tho. what do you think? funny it looks like it has 2 stems!

I'm guessing either Mycenea pura, or more likely in my mind something along the lines of Mycena stipata. First one might be a Micromphale sp. but it kinda resembles a Xeromphalina sp. Second one kinda resembles Laccaria sp. Wish I was better at the ID's, but nice pics none the less.
-------------------- Having lived through an existence close to nature, one accepts the small and simple things as most important in life. Sun, wind, rain and snow. The sounds birds make, smells of fresh wild flowers. Love of all kinds, from friends and family, thy self and our neighbors. Beautiful sunrises to the darkest clouds dancing above in the sky. To forgive, learn, share and express. This is the only thing a man such as myself can ask for. What comes as the result is nothing short of the core of human existence, to truly live free in body and mind.

1 2 both Xeromphalina, an accurate ID to species can be tough without more info as they all look basically the same http://www.svims.ca/council/Xeromp.htm#nTab 3 Flammulina, tree species? 4 not a blue but cool stem

Nice little adventure you had there.
I too have seen thick, double-like stems on Mycena before.
