Plant disease. Help!

Which plant disease is it? It looks like white holes in the leaves. Is this an insect eating the leaves? What can I do?

I'm guessing this is a pereskiopsis?
Looks to be bug damage. Could be thrips or something else.
I find this sort of damage to be seasonal and so long as they don't chew out the apical meristem, it will continue to grow. If you feel you need to do something about it you could use a deterrent spray like chilli, garlic and oil mixed together. Or if you are dead set serious about ridding them you can use a systemic insecticide, although that's a harsh road to go down. Try other methods first.

The pereskiopsis will not grow anymore, because it is grafted. I guess I have to use insecticide. The annoying this, is I have already used insecticide about 4 months ago. But then I think it was another kind of insect. The insecticide is also a bit harmfull to the plants.

I'm fairly certain that it's not scale.
@jivangilad: Try and source a less harmful insecticide like White oil, Neem oil, pyrethrum or make your own from one of the numerous recipes available online.
Bugs are seasonal and you might get spider mites in hot, dry weather but when that subsides you will have to deal with caterpillars, white fly and in the cooler wetter months, slugs, snails, earwigs and other such critters that find your crops tasty. This is why it's important to be vigilant and check your plants regularly for pests. This way you can see if any damage that is occurring and deal with it before it becomes an infestation. If you're growing plants, pests will be an issue and you have to learn to deal with them as they arise.
I check all my plants at least 3 times a week, more often if I have time. Even doing this I still get damage but a little damage is ok and the plants can handle this. Infestations are what weakens the plant and can kill it in severe cases.
