I apologize in advance for this beginners question but when people say they do 1 Qt spawn to whatever amount of substrate do they actually mean 1 full Qt or do they mean 3/4 or however full their quart jar is?

I usually mean 3/4 or however full my quart jar is. I've never had a problem. Though if you want to play it safe adjust for that and use the appropriate ratio for the substrate you're spawning to.

Just be clean. If the bulk substrate is full of trichoderma spores a 1/1 bulk/spawn ratio is not enough; if it's sterile one cell will eventually conquer the bulk substrate.
