Extremely early cap formation / growth termination

I have a tub of Koh Sumui Super Strain and a tub of Orissa Indias which are both showing awesomely even pinning and great early stage growth, but 2-3 days after the fruits form the caps begin to break from the stem, terminating growth and leaving the fruits no longer than an inch. I recently switched casing soil due to availability, and am now using Miracle Gro potting soil w/ extended release plant food. The total nitrogen is .21%, soluble potash .14%, and available phosphate .07%. The extended release part is that a portion of each of these minerals are coated to release slowly over time. Could this casing soil be why the fruits are growing so small?
The only other thing I can think of is that I originally inoculated some of my jars with Treasure Coast, but after a few weeks had no signs of growth, mycelial or contam. So I ended up re-inoculating some with Orissa India or Koh Sumui (not together obviously). I thought that because there was no sign of growth from the Treasure Coast species, it was still sterile grain so still be viable for other species. Could this be a reason why my fruits are so small?
Fruiting temp has been hovering around 73F, lights on 12hr cycle, fanning each tub a few times a day, polyfilled holes w/ fan in the same room. Edit: should add that the sub is coco coir colonized w/ birdseed from jars, and that humidity is high enough to keep moisture on the walls. I have never had issues with this set up before.

this is extremely common with koh samui, it happened practically every time I grew it. A few people seem to get decent results with it but I always have a useless flush of shrooms that arent even worth harvesting.

I think it is a mutation caused by a contam. Just one theory... It happened with a trich contam. Causes the strongest mycelia to have mushrooms that miniaturize... Totally sucks. Mushroom mycelium does weird things around contams

really? I've never heard that about Koh Sumui, but I guess I hadn't really looked into it either. The confounding issue here is that this stunted growth also occurred with a tub of Orissa Indias...

@ Raw: ugh I was a bit worried about that as well... There doesn't appear to be any visible contam of any kind but you're saying it could be a mycelial contaminate, something that could allow great mycelium but cause tiny fruits?
