Help with harvesting the fruits!

Hey guys, this is my post ever.
I've been successful thus far with my first grow, but i've run into a problem that requires a little help.
Started to harvest a couple mushrooms today, but every time a mushroom is picked it takes a chunk out of the casing/substrate.  How can I avoid this so that the substrate doesn't have craters after?
I've tried twisting but it still takes a bunch of mycelium out with it... There's gotta be a way to preserve it for further flushes!

I'll also add that the substrate is a 60/40 coir-verm mix

I've read that some use a sterile scalpel to cut them at the base.

It's always going to happen unless you cut the mushroom at the base with a pair of scissors or somesuch, but then you'll leave behind rotting mushroom matter. Just make craters getting them out and patch the holes up with more clean casing layer.

Ok, so i'm assuming it would make sense to patch it up after I dunk, just prior to the second flush, correct?
And i were to use a scalpel to cut at the base, then no mushrooms would grow on that spot on subsequent flushes?

IDK about the casing, I just leave the holes...

That spot may mold/contam or the mushrooms may eat it & use it as nutrients...
