Ok so here is my set up. I have a 10 gallon fish tank with a fiberglass lid i cut to fit snugly on the top with a small hole for air exchange. The bottom is lined with wire for the cakes to sit on, there is a humidifier putting humidity into the tank through a hose. Two blue lights are at the top.
Well everything seemed to be going fine, the mycelium caked well, then I waited a week, dunked and rolled them. then put them in the tank.
Ive been misting them a few times a day, and doing air exchange as well every other day or so.
The temperature stays around 70-80 degrees, and the humidity fluctuates between 70% and 100%
2 mushrooms successfully grew after a week and a half, but then after that nothing.... only a couple pins formed, and only 2 of them grew. I cant even see the other pins anymore :/
what happened?

70% humidity is not enough for those PF Cakes. You need mid to upper 90's.
Build a shotgun terrarium. They're super easy to make, and pretty cheap. http://mushroomvideos.com http://www.mushroomvideos.com/Terrarium-Tek Part 3 is where he starts to build it.
1/4" holes every 2 inches.

what if i just put some per-lite in the bottom? a few inches of it. And drilled more holes in the top for air exchange?
also what about lighting... I keep those blue lights on 24/7, is that ok?

ive noticed my grows always do their heavy lifting during 100% darkness. mushrooms just love growing in the dark. try to get your room that way 12 hours a day or so and your little friends will love it. and as previous poster(s) have stated, 70% humidity is a bit too low, you want 95-100 for cakes.
