This file is intended to list all the effective psychoactive plants which are also known to be relatively safe. All the botanicals listed are either overtly psychoactive or have historical significance, or both. I’ve attempted to pinpoint the main species in order to keep it simple, but there may be related species in any given genera which have similar effects.

I like it. I'm working on a personal list similar to yours, though I've got all the common names + basic info + dosage. Dosages, as pointed out to me by several regulars, should be kept out of public listings.
Here's an example from my list.
Calea ternifolia [Calea zacatechichi, Dream Herb, Cheech, Bitter Grass] HALLUCINOGEN Edible/Smokeable Leaves, Flowers The Chontal medicine men, who assert that this plant is capable of "clarifying the senses" causing euphoria, call it thle-pela-kano, meaning "leaf of God". Whenever they desire to know the cause of an illness or the location of a distant or lost person, the common ritual is to smoke a cigarette, whilst drinking a tea, both made of Calea Zacatechichi, right before going to sleep. Some also report placing the leaf of God under their pillow before sleeping. Reportedly, the answer to the question comes in a dream.

I one day plan to expand this list into an entire book listing effects, etc. for each botanical.
