Sounds like the mushrooms are dieing either from lack of air or too little himidity. If it was my guess I would say your humidity is too low.

Hey man the situation you described is exactly what happened to me. The way i fixed it was i hooked up a small aquarium bubbler and bubbled it through the bottom of the perlite. It worked awesome, it eliminated the build up of CO2, just make sure you have a tiny hole in your terrarium for the air to escape from. Also don't set your cakes directly on the perlite unless they look dry,
