Mono Tub Green Mold....Question?

A friend of mine is pretty new to mushroom cultivation. He's had success with cakes and casings so he decided to try a monotub out. He used rye seed and brf cakes (fully colonized with amazon strain cubensis) to spawn with 3 liters of horse poo, vermiculite, coir, and gypsum. Everything colonized beautifully. After full colonization, he pulled the blanket off the tub to natural light and room temps. Having six 1 and a half to 2 inch holes on the sides of the tubs, he didn't fan. Instead, he had a fan running (not facing the tub) with the idea that the air circulating around the room (200 square feet) would provide sufficient air exchange through the holes (stuffed with polyfill). On the 4th day of fruiting conditions he noticed dense mycelium in a small area that didn't resemble the rest of the mycelium. On the 5th day he noticed green mold. The whole project was abandoned and the substrate laid outside in the shade (minus the green mold patch) in hopes of mother nature providing a miracle.
My friend was wondering what happened? How might this be avoided next time? Any feedback would be greatly appreciated. Thanks for the time everyone.
