Needed:
Brown rice flour - This is easily obtained at any Bulk Barn, or you can buy brown rice at the groccery store and powder it yourself at home with a blender.
Vermiculite - This is sold at garden stores, hardware stores, some super-stores (eg, walmart, zellers). Look for it in the gardening section, if you have trouble locating it simply ask a store assistant and tell them that you are growing indoor plants.
1/2 pint canning Jars - These are sold at lots of places, such as hardware stores, wal-mart and even some groccery stores sell them. You are gonna want ones which are shoulder-less, which basically means that the jar is wider at the top than at the bottom. That, or jars which have a uniform width( top and bottom are same width). You need these so you can get your cakes out in one piece.
Aluminum foil - This is for sterilizing as to prevent water from splashing/dripping into the jars upsetting the water content.
Large pot for steaming - Contrary to popular belief, a pressure cooker is not required for sterilizing PF jars. A large pot with a good fitting lid is more than sufficient. You'll also need something to suspend the jars above the bottom of the pot as to prevent the jars cracking from heat, drying out the substrate, and/or upsetting the water balance.
Rubbermain container - Doesn't have to be that brand, any tub large enough to fit all your cakes into when placed side by side works good.
Perlite - This is to provide humidity to the mushrooms during the fruiting stage
Incubator (optional) - This is optional because unless the area your jars are sitting in is less than 60 degrees farhenheit, you do not need one. Higher temps WILL make the jars finish a day or two early, but it also increases the likelihood of a contaminate gaining the upper hand.
A couple light bulbs - Go to a hardware store or wal-mart or whatever, and look for what's called a Compact Flourescent Bulb (CFL). You want one which has a temperature spectral range of 5500-7000 K (kelvin). Some brands print this on there, so keep an eye out. Most others will put something like "daylight" or such. That is the one you want. Incandescent bulbs are not recommended.
Desiccant - This is for drying the mushrooms once you've grown them. Desiccant is a material which absorbs water readily and is sold in some hardware stores. The best is Damprid, and is very effective, easy to regenerate, but is expensive.
Another alternative is stuff called "moisture magnet". It's what I use and it does the job good. It's about $7 for a 1 kilogram bag.
Fan (optional) - I like to split my mushroom's stem in half, than fan dry them for 1 day. Then I throw them in the desiccant chamber and they are nice and dry in 3 days.
There's a couple few things you'll need if you want to make a spore print.

2 tons brf 5 tons verm 500 gallons water 3000 jars
Seriously though, if you just buy an averaged sized bag of brf and a medium sized bag of vermiculite and one case of 12 half pint jars, you'll be set for at least a few grows.
Here's the exact numbers:
"For one ½ pint jar(~240 ml) you will need: => 140 ml vermiculite => 40 ml brown rice flour => some vermiculite to fill the jar to the top (app. 20 ml) => water
For 6 jars, this amounts to: => 3.5 US cups vermiculite => 1 US cup brown rice flour "
Source: http://www.fungifun.org/English/Pftek
Other than that, get a drill and 1/4-1/2 inch drill bit, a large clear tub, and a big bag of perlite and you'll be good to go.
Joneveryman beat me to a more exhaustive list though, so read what he said too.

I've used miracle grow perlite, and was sketched out by the fact that it had miracle grow in it, but no one's had any weird convulsions or anything from it, so I think it should be fine. We did experience a great deal of nausea, but that's normal.

Ill ditch it and get some new regular perlite. When will the stores start to stock up on these items for the spring season?

I don't know...I haven't used perlite in quite a while. Or verm. Or BRF.
But some stores carry overstock from last year so you should be able to get some.
