Ya, so it looks like it is growing ok, but its got some sort of hole in it. It looks kind of nasty jagged. What do you think? I saw on some random thread similar to this one that said it could be that it isn't getting enough water. Anything else? Thanks!

A lot of times if the humidity is to low the tops will crack in a star like fashion, never seen it look like that though. Kinda hard to tell with the pictures.

Sorry about the picture quality, the most sophisticated camera I own is my phone. I'm leaning towards this answer because the hole in the cap did look star-like at first and I have not watered the perlite in a while.

I did not see any black anywhere. The inside of the hole looks fine, like a cross section of the cap.

That was another possibility I considered. If you search "exploding caps", you can find a good thread with pics of some pretty nasty looking mushrooms.

If you don't see any Green or Blackness in or around the jagged edges then you should be fine, Looks like it is just a freak mutant 
But if you want to be extra care full you could always just remove that one mushroom and destroy it so there is no chance of it spreading to the others, but i doubt that will be required. It looks near fine.

The caps splittiing like that and some mutants are caused by rapid changes in humidity.
Your humidity is up and your FAE is good, otherwise your specimens wouldn't be growing. Then when you mist and fan, parts of the FC stay drier and take a bit longer to build up that humidity again. The mushrooms are desperately looking for that humid fresh air and when they find it they grow, when its absent they tend to want to stop growing.
Its like you are playing red-light green-light with the mushrooms when this happens. High humidity and fresh air for a couple hours, then low humidity and fresh air for a couple hours.
Finding that balance is key to perfect mushrooms IMO.
btw there is nothing wrong with having mutants. They still want to be consumed!
