Black Pin developing near 100% colonization

I've re-ordered some jars online, and also syringes from Sporeworks, and am trying to start up after my last attempt stalled out and I was unable to continue for the holiday season.

I had a few Pint jars which went relatively quickly from multispore inoculation; however, all of the jars stalled out at the very tip-top center of the grain. They'd colonize to about 96%, the entire jar a bright solid, healthy white - but upon close inspection you can see a few grains on top which never colonized and developed black pin mold. Just a few, mind you - like some brown nuggets on a snowy mountain.

So I'm confused. How could the jars quickly colonize all the way to the last moments of near-success, and then suddenly develop black pin mold on the top few grains?

The jars were shaken at 30%, and recovered quickly and kept going. So it had to have developed later on. They had a nice thick tyvex filter on top with no punctures, and was never wet or anything that could have caused a contamination breach.

I'm really confused as to how things happened that way.
