I hate to bother, but I am having some ID trouble once again.
#1 No spores came out for spore print, growing on live tree in bottomland hardwood (mostly oak) forest, cap diameter 3-7cm, flesh very tough.
#2 White spore print, growing on rotting log, no stalk, cap 5cm diameter, habitat bottomland hardwood (mostly oak) forest.
#3 This is one of the few mushrooms I have seen growing on the ground this November here in WI, and it seems to be rather abundant at least locally. Spore print white, stalk length 7cm, cap diameter 9-11cm, growing in hardwood forest floor(oak/aspen).
