In my experience, the top vectors of bacterial contamination are:
Improper Sterile Technique - You need to use latex gloves (after washing hands and forearms with anti-bacterial soap) and tyvek wrist sleeves when inoculating, rubbed down with alcohol. You also need to use a glove box and flame sterilize your syringe. Next to mold spores floating in the air, the CULTIVATOR is the biggest source of contamination. Bacteria is all over your hands, fingernails, hair, and in every breath you exhale.
Incubators - Colonizing above room temperature only favors bacteria and competitor molds, not mushroom mycelium.
