1) stop using the incubator, just put them someplace warm. And not 'airtight'.
2) Grain prep. Try the method shown in Let's Grow Mushrooms. You'll get a better, more usable grain.
3) Spore source?
4) sounds like bacillus.

Well, I'd want a pic. But if it just grows around the edges of the grain and doesn't go anywhere there's some sort of issue, and the most common is probably water balance or contamination.

Have you grown cubensis or any other mushrooms? Cakes or grains?

mexicana and cubensis are species, not strains.
But yeah- if you can grow on cakes and fail on grains, then it's probably your grain technique.
