I like to keep mine so that the top is aimed at sunlight, but the base is lightly shaded.

This I've found ensures no etiolation, like is going on there.

Sadly if it is already on that course it will need to fatten back out again, but it will have that skinny section for a long time

As far as the pup, try giving it a shot of ferts to see if that will help the growth.

it's normal for cacti to grow skinny when they are in the process of forming a decent root system.
there is no way around this.
once established they will fatten up again.

how long has it been potted?
if it has been a while then you should check for roots.

don't water it until you know that it has rooted.

Here take a look at this growth, 2 months old. This was a very thick tip cut of an old log which I put in the ground with no roots it started growing very fast in the sun but was thin as you can see it quickly starts to gain it thickness, nothing to worry about.

Eventually as the plant grows you may wanna bury the thin section for better support just as you would with seedlings.
I would also use a few inches of sand around the thin section for a while because the skin there is not hardened off and moisture from soil could cause rot. Over time from getting no light that section of cactus will callous.
