Could be many things:

Mutations
Growing Conditions -- (humidity, depth, actual field capacity of substrate) FAE
Whether its an isolate or multispore injection.

Not sure, mate, but looks to me like a bit of high humidity? Just a guess.

They're aborts.  There is no clear answer that I can find.  It could be too little humidity, too much humidity, too little water in the substrate, too much water in the substrate, a property of the strain, or who knows what.  What is your exact set up?

It aborted. It could be a lack of F.A.E, low R.H, insufficient nutrients (unlikely if its the 1st flush) or lack of moisture in the substrate.

Or it could just be some crappy genetics.

need some more information to determine what exactly may be causing this, have you took a humidity/temperature reading and on average where does it sit?

what flush is this?

around my 4ish flush, i get them on both trays of sub, one was dunked around 3 weeks ago and the other 9 days ago, i have the shotgun terrarium with perlite, and after i fan...5x or more daily i hit it with the ultra sonic mister to get the humidity back up, while occasionally turning the ultrasonic on throughout the day for 5 mins. and yes it was multispore innoc. my hygrometer is on avg 75* 95%rh

Given that it's the fourth flush, it's probably depleted nutrients.  I'd say kill it after this flush.

libertaire is right, after the 2nd flush you'll start seeing more and more aborts, your mycelium is running low on nutrients.

Most people won't push their substrates past the 3rd flush since at that point the mycelium will have weakened to a point that mold can set in.
