Habitat:
Found growing on a dead piece of wood. Illinois (U.S.)

Gills:
white, not attached, gills

Stem:
Length about 3inch, about 2mm think, color = white, texture = fine, hollow, thin.

Cap:
Diameter = 1 inch, color = greyish/white, texture = fine, UFO flat like.

Spore print color:
whiteish greyish

Bruising:
none

Other information:
Scent of the mushroom is usual.

Well, that one's a mystery.  The only mushrooms I can think of with free gills are Amanitas, Agaricus, Pluteus, Coprinus or Lepiotas.

Maybe it's a small Lepiota of some kind.

Lepiota should not be growing directly from wood.  That would be my guess though.
