I became convinced that that reality was the shape of a sea shell once. With the big bang starting at the point, and time and the three spacial dimensions made up of the shell itself, expanding along time and kept in the spiral shape by the universes own gravity.
Theoretical physics is an amazing thing to wonder on while tripping.

Um. One time I was tripping at my dads house when they were all on vacation. His living room was one where there is no floor above it so the ceiling is all the way up on the roof. And there was this ceiling fan that is hanging down from the ceiling. When I was tripping on shrooms I was positive that it had turned into some sort of plant like creature that had a big eye in the middle of it and it was planning to come and swallow me.
That's probably the craziest thing I've seen while tripping.
Also on the same trip for some reason when I was watching TV and anyone was talking while on screen it sounded like they were talking backwards. But if someone was talking and I couldn't see them, than I could understand them. This made watching sports center scary as fuck since the set was all crazy, I thought the whoever was on at the time was the devil.

The most intense visual I've ever experienced was on 3 hits of really good acid, a year or so ago. I was in the room I grew up in as a child, with a few of my friends. No one was talking - words no longer existed. We were all just sitting in the room, psychedelic lighting abound, listening to A Perfect Circle.
3 Libras came on, and my god... the most beautiful thing I've ever experienced. As the chorus exploded (And I threw you the obvious, to see what occurs behind the eyes of a fallen angel, eyes of a tragedy...), a vision of a woman (Gaia) appeared behind my eyelids. Her gentle eyes were closed, and as the music really peaked a plethora of life exploded from behind her, of flowers and animals and endless color.
I don't think I've ever felt so .. at peace. The experience stuck with me for a long time.
I hope to see her again one day... because I feel that she is always with me, guiding me on the path.

The most recent crazy trip with sally
The other night I smoked some salvia with a few unknown friends ..I had some of the coolest CEV's ever .. Almost like my first mushroom trip
Golden leaves would spiral then flutter around what appeared to be blue and green cartoon grass ..Almost felt like wind was blowing .. A look at the wallpaper showed an upside down mushroom turning into a top hat then slowly revealing a female face
My best salvia experience yet ..

I'm pretty sure that seeing Infinity is the craziest thing I've personally experienced with any psychedelic.
Seeing the timeline that I'm supposed to follow, and being pulled farther and farther away from it all within my mind. Still materially there, but mentally forever away.
Ok, maybe this isn't the type of "seen" you're looking for... Hmmm... OH, I KNOW! 
Whilst on Acid in Mexico, I went to the faucet to pour some water... And when I turned on the water, what came out was a surprise. Colors of green and blue and red and orange poured out. Every surface in the kitchen began to glimmer like this rainbow that had poured out of the tap. It spread, to every surface in the house! Soon enough, the living room turned orange and purple, and my friend turned red. The colors were everywhere, growing, intensifying! And rainbows filled my vision- there was no escape from the colors! And I loved every minute of it...
I went on to redose a few hours later as I sat on a sandbar as the sun came up and the tide went down and smoked a bowl. The visuals followed me back across an international border and all the way back home, and eventually my 20 hour trip came to a relaxing close. Though I'll tell you, riding back in my convertible (my friend was driving) was AMAZING tripping my ass off. All that cacti and those clouds and mountains, crawling and waving and rolling along...

Anno transforming into an octapus or some kind of sea monster on the beach, and his entire body inflating to the point where i was thinking he's gonna get up in the sky like a baloon!
But that was after an ultra potent tea he made out of 4-5 different strains and a small mistake in dosage calculation.
Later on, the waves where trying to bite me...
Can't forget those tentacles comin' out of the clouds, nor will i ever forget the green sunset over the purple sun.

the mothman from the movie "mothman prophecies" started communicating to me through the TV, it told me to go outside and sit in the middle of a field in a plastic garden chair.
i went outside and sat in the summer breeze and the trees took the forms of gods. it was dark but the silhouettes were visible. they danced in the wind, then turned to physical fractal images.
another time i sat in the same spot during the day, staring at the forest and the elephant-headed hindu god stepped out, only it was in a skeletal form, it had swamps and dead time as clothes and we were laughing telepathically. it just stood there, later i saw it was a brighter tree because it was dead and its bark had fallen off.
i've seen carpet flowing out of the carpet endlessly, beer foaming in the the ceiling, me growing a neck that's three feet long, a toilet turned into a cathedral lined with gold and ruby plates and much more.
i also saw what thoughts do when nobody is thinking of them.

somtimes i get scared to trip cuz theres a movie called shrooms which never made it to usa an its about these people who trip in the woods an this thing comes out to get them some weir creature a man like just ugly they only see him when they trip. check it out really trippy.

I couldn't tell you the craziest shit, because it's been too crazy.
But I remember seeing my friends faces and they were like skulls (not really, it's kind of hard to explain). I was noticing the bone structure in their face instead of the skin, eyes and lips. This kind of created the illusion that I was looking at the shape of their skulls.
Then, the skulls would kind of "breathe out" in the way things do while you're tripping, and slowly the bone structure would fade until I was noticing their whole face again. Then slowly the bone structure would be come more and more apparent, then breathe out into a face again.
Around their eyes I would see those giant black holes where eyes go in skulls, I'd see their cranium instead of the hair on their head and I'd see all the bones making up their jaw instead of a chin.
It was fucking amazing.

Let me think... one time on Mushrooms everything smeared a pallete of multi colored gold plaid patches on my vision at the moment of ego-lessness and my face went like THHIIIIIISSSS BIG and some words flowed out of my mouth like some really sweet wine and it capped off the fleeting pallete of gold colors... then I watched a stream with my friends for a few minutes as we saw the life force of some sort of mass energy in the water.
BTW, I'm sorry but that Shroomz movie... utterly hilarious. HILARIOUS

I always see yantras when I'm tripping. Even before I knew what they were, I'd always have those exact triangular patterns float around, and distort whatever I was looking at behind them. Also, weird little "shroom monsters" that float around my closed eye visuals. Random beings, trippy things like a baby lizard that EXPLODES into a dragon for a split second as it strikes/bites and then returns back to the innocent, little cutie. Random sets of jaws, teeth, eyeballs, skulls, "mental fish" swimming around my CEVs.
