Lost my notes, could ya'll fill in the blanks?

So I haven't been actively growing mushrooms lately because it's been too cold. During my time of inactivity I reformatted my computer's harddrive and forgot to backup my mushroom cultivation notes 
Rather than trolling the forums for hours to recompile what I had, could you guys maybe dig into your notes and post the recipes below:
1. Ratio for sawdust pellets, bran, and gypsum mixture. 2. Malt Extract Agar recipe (I am pretty sure the only ingredients were malt extract, agar, and water) 3. Malt Extract liquid culture formula (two ingredients: water and malt extract)
Those should get me started again. Thanks!

1. 10% of each by volume 2. 10g Dry Malt Extract, 10g agar and 500ml water 3. 1/2 teaspoon Dry Malt Extract (or honey) and 500ml water

I've read on here where others use 1 Tablespoon DME per cup of water. That would be like 12x the amount that you use. Has anyone ever compared growth rates based on carb content in lc?

Yeah... I've heard that some people use more, but I've done it both ways and the 1/2 teaspoon works very well. The idea is you don't want a LC to have more sugar content than the grain you put it into. That way when the mycelium hits the more nutrition, sugar full grain it goes into hyperactive growth rather than slowing down. I originally got the recipe from tahoe's LC tek.
