How real can psychotic delusions be?

Partly inspired by a guy who comes into our store with strange conspiracy theories way beyond the most far out things you read in these forums, and partly by watching madness portrayed in movies like American Psycho, Fight Club, A Beautiful Mind and Proof, I'm just wondering how real delusions can be.
Do advanced schizophrenics really see things and people that aren't there, or is it just that they interpret reality in a distorted mixed up paranoid way? When they say that they have important high paying jobs, do they really believe that and vividly hallucinate that reality, or do they simply say it because they don't know who they are any more and want to cover it by trying to sound important?

I know someone who has delusions that people are coming into his apartment while he is asleep and/or gone. He has elaborate scenarios of how this is all happening. They get his voice mail and know when he is out of the house. They sneak in the back door when he opens the front door, so they don't trip the alarm. They poison his food so he has to eat at restaurants all the time.
He isn't faking it. He only does this when off medication.

My grandmother is schizophrenic and she does many of the same things the OP talked about, believes people are sneaking into her house, thinks there is a 5 foot tall bird which tells her to do things etc...
It seems to me that she actually doesn't see these things outside of her, but it's all taking place in her head.

My grandmother did wacky things, but she isn't schizophrenic. She used to talk to a plant and she thought my mom was someone who looked like my mom, but really wasn't.

I have heard of that. It's the Capgras Delusion.
Since posting the OP I have been reading posts on this site which coincidentally (or is it? ) was linked to in the Pub, and it almost appears that some do not only hear but see menacing figures when off their meds.

She'll point to the bird in the room and actually get scared, but I don't think she visually sees the bird. I'm pretty sure the bird is just inside her head.
But yeah, the bird talks and everything. We asked what kind of bird it is and she says it's a robin.

100% real. Insight may be gained into psychotic delusions by undertaking a dissociative binge; re: Lilly.
