Yeah, that would make an excellent substrate.  I would do gypsum at 10%, verm at 20%, and the others in any amount.

Yeah, that'd work great.  You don't have to worry much about the levels of coco or coffee, as both can be used as subs on their own.  I just add any amount of either.

Gypsum is calcium and sulphur, both essential.  It's supposed to up your lids a bit.  No need for oyster shell, gypsum has your calcium needs covered and won't impose much affect on pH, as the shells would.

No prob.  Are you growing straight in the tubs? There's a good chance you don't need to use a humidifier.  I would just cut some holes in the tub and fill it with sub to 4 inches and simply mist as/if needed.  Often you can get a great tub without misting at all.

Okay, just don't over-saturate your grow using the humidifier.  Good call on the casing, it's optional, and unnecessary in my opinion when dealing with cubes.  I used one the first few times but have long since abandoned the act.

Yeah, and the casing gets stuck to the shrooms, making the harvest more of a hassle IMO.

I don't know, I don't partake in the meat biz so I don't use bone or blood.  I don't know about guano, I've never used it in a sub.  But there's no need for those as I see it, you've already got a prime sub using those other ingredients.  Just try to nail the other aspects of the grow and it will kick ass, genetics permitting.

Just did a quick search on using guano in a sub - the word is not to use any carnivore shit.  It's way too hot for your substrate.
