I have experienced Ego Death on my last LSD trip which was a 6 tab LSD trip (I am very experienced with LSD). I have only had 1 mushroom trip which was 4 grams of pure Cubenesis caps, and experienced Christ level thought processes and could feel everyone on Earth at once, I felt God -- that's an entirely different story though.
As for the ego death, I reached it accidentally, I did not intend to do it because it was literally the scariest experience of my life. If you are looking for "ego death" you surely will not find it. Because your ego is involved in "looking" for it. I truly believe it just happens, when you are ready for it.
How did I achieve it? I was meditating on the thought of "Let Go" ... and then I thought I was being sucked out of this reality, I thought I was dieing. I forgot who I was, I did not even know my name. I forgot where I was. I scrambled to write down my name which I didn't even do because of my mission; meditating on "Let Go". Paradoxically this fear of being "sucked out of this reality" made me so happy because I realized so much. [The visuals during this time were INTENSE and I nearly vomited -- saw cosmos forming, planets forming etc. real f*cked up shit]
During ego death, you realize all your little games, all your little thought processes and it gets shattered, who you think you are IS NOT WHO YOU ARE. You realize your true being. Your inner Buddha, it flowers and opens you. It was a very enlightening experience for me.
I have not had the opportunity to have LSD stumble upon me since that last trip, but I have a feeling it will come -- I am not looking for it though, but gladly welcome it on my tongue 
My advice, if you want to experience ego death, get LSD. (I'm not too experienced with Mushrooms -- only done it once)
I am sure mushrooms can show you a lot -- they showed me a lot, but I am not sure if you can experience "ego death" on mushrooms. When I was tripping I was so lethargic ... like my eyes were all fucked up and my body felt so disoriented and mentally I was so confused, or felt confused.
My advice, meditate and look within yourself while tripping... let the mushroom take you over for that time period. Just think of "Let Go". Whatever thought comes up -- nope -- "Let Go".
I wish you the best and hope you do experience "ego death".

I haven't even experienced full ego loss yet. I was at the door, and I was trying to push myself out.
But the LSD would not let me :[

GREAT post, Voidking, that was highly entertaining. I was looking for story experiences like that when I started this thread. Unfortunately, I wouldn't have the slightest clue as to where to get LSD and I certainly don't have the lab skillz to synthesize it.
I get that I need to let go and stop concentrating on "yeah, I'm gonna experience ego death!". The experience posts are particularly helpful for this.

Describing ego-death would be like trying to describe your sense of vision to a blind person, or colors to someone suffering daltonism... U can't really put any words on it. As for ego-death, you just have to feel the flow.

ilike that ^ you cant really describe it you cant prepare for it well.. it just hapans. and if ur out seeking it i usualy dont get it. ive only had ego loss when i wasnt intending to get it

Yea man. For real though, meditate and concentrate on your breath. Whenever a thought comes up -- "Let Go"
When I did this, each breath I took was deep and I got higher and higher and higher until finally my mind shattered
