 Double shroom/Reversed cap! Pics

The last flush on my Keppers CReepers casing was very good.
In the midst of the sea of shrooms I found a weird shroom with a upside-down cap on top of the cap of the shroom.
It's not supposed to look like this, I know.
Have anyone seen this before?
