Im buying her a tool t-shirt, pretty sure shes a small but i dunno anymore, dont know how to compare the sizes of a normal shirt with the (s,m,l) of band t-shirts.
anyways shes 5'4, 137.9 pounds (we weighed today ;p), 32-34/DD

In a ladies' size like that, I would order a Medium. A small would fit someone with a A or B, maybe pushing a C cup. A medium will probably be comfortably snug around the bust.

I suggest buying a medium and keep the receipt in case it's too big/small.

I'd go with a medium. Chick's band shirts tend to run short (why???) and if she's got tits, which apparently she does, they're going to pull it up and a small is going to be a 80's Madonna cropped top probably.
Why the hell don't they put a sizing chart on their site...or at least a picture of the actual shirt instead of a "representative drawing?"
I'm assuming she's rather curvy (and I am NOT using that as a euphemism for FAT, I'm using it in the CORRECT way, to mean she's probably got a disparate chest-waist-hip ratio)...
It doesn't look like it has much inward-waist-curvature which might make it sort of like a tent that starts at her boobs and goes straight downwards, thus being highly unflattering at any size. Either so tight in the chest it flattens out the goods or so loose in the waist it falsely thickens, either way creating a square figure.
Or it might be great, since that's not an actual picture of the shirt.

Ooh, wait, what does "Tissue" shirt mean?
This could be an important investigative tool. If it's the company that makes the actual shirt, there may be a sizing chart somewhere. If you find a bra sizing chart, you can figure out the largest measurement around her chest (unless you already know, which you probably do) and use that.
If they mean it is a very thin material, that changes the whole ballgame. Thin shirts are way more flattering to boob/waist ratios, bacause they are more stretchy, so you might could go with a smaller size for her waist and let it stretch over her boobs. Of course, then you've also got to think about the shoulder width...
Shopping online is such a bitch.

It looks a lot more "curved" than I originally thought, but it also looks short. And it's hard to tell because the mannequin has the boobs of a boy.

I'm around the same size, just a little smaller and my boobs aren't that big. I'd probably go with a medium.
